Please use this form to submit a mission request.  Please understand that all members of the

Patriot Guard are 100% volunteers and are not available at all times. 

Rest assured that we take your request very seriously and will reply as soon as we possibly can.

The PGR attends missions at the direct request of the family,

or someone representing the families wishes

(I.E. Funeral Director). If you are not a family member,

please include the family contact info in the comment box below.
